At Perimeter Home Health, we have licensed professionals that work with your doctor to ensure that we provide you the best quality of care.
Skilled Nursing
Monitors patient’s vitals. Provides wound care. Assists with medication management and much more.
Physical Therapy
Helps patient adapt with using assistive devices such as walkers, wheelchairs. Regain prior level of function by teaching / supervising strengthening activities.
Occupational Therapy
Focuses on patient’s activities of daily living. Helping patients complete daily tasks such as bathing, dressing, cooking. Identifies hazards in the home environment and optimizes for patient safety.
Speech Therapy
Primarily concerned with the patient’s swallowing, language and cognition issues. Speech Therapists play an important role in treating patients having difficulty swallowing food and/or liquid safely, speech and language issues related to stroke or TBI, and strategies to overcome or adapt to cognitive impairments.
Home Health Aides
Provides extra help for patients that need a little extra support with activities of daily living. Though our goal is to ensure that the patients become independent, sometimes recovery just takes a little bit longer and a Home Health Aide could help with bathing and grooming tasks.
Medical Social Worker
Addresses social and financial aspects of patients rehabilitation. Helps patient navigate services that may be available to them to help in their recovery.
How much do our services cost?
If your primary insurance is Traditional Medicare
Since we are a Medicare certified skilled home health company, Medicare will pay 100% of the home health care services that you receive from us. No cost to you.
If there are services that are not covered by the Medicare home health benefit, we will notify you in advance verbally and in writing before charging you for those services.
If your primary insurance is a Medicare Advantage Plan, Commercial Plan, Medicaid

Although we strive to verify and give you accurate information of the cost at the beginning of your care, your insurance company sets the deductibles, co-pays or co-insurance that you will have to pay after we render our services. Each insurance plan is different and the patient financial responsibility differs for each plan. We recommend that you call your insurance company for the most up to date information.
